Nurture Care’s Person-Centered Plan Facilitation Training
Nurture Care’s Person-Centered Plan Facilitation training is a four-month comprehensive program designed to teach essential planning methods and facilitation skills. These include MAPS, PATH, Liberty Plans, Solution Circles, building community capacity with Learning Journeys, Person-Centered Thinking discovery, and more.
The program begins with a six-session virtual foundation in facilitation, providing a solid groundwork for participants. This is followed by group mentoring sessions to build confidence and refine skills. Facilitating plans can feel challenging at first, but this series allows you to learn, practice, and grow under the expert guidance of experienced plan facilitators. After completing the six-session facilitation training, participants will engage in six 1.5-hour mentor sessions. These sessions dive deeper into advanced planning processes, tools for discovery conversations, and writing meaningful summaries based on what is learned during person-centered planning. Participants will also benefit from peer reviews, mentor guidance, and direct observation during individual mentoring sessions.
This program also explores how effective planning can guide teams and fulfill licensing requirements in a more impactful way.
