At Nurture Care, we use Person-Centered Planning processes that empower people to create a life that is meaningful to them. We believe that all people should have positive control in their lives. Nurture Care’s supports and planning focus on the person and their needs and put them in charge of defining the direction for their lives. A strong, committed team is the key to a successful plan. We engage all team members important to the person.

Nurture Care facilitators and graphic recorders have been trained in a variety of tools and methods including PATH, MAPS, Liberty Planning, Life Moves, Picture of a Life, building community capacity using the Learning Journey tool, and more. Our facilitator will work with the team to determine the process or group of processes that are the best match for the person and their vision of the future.

Person-Centered Planning can be used in place of a traditional planning meeting to help people move to a more meaningful future. Planning can be used when there has been a significant life event, when people are ready to move, get a new job, connect with the community, build relationships, and many other situations. The Person-Centered Planning methods are tools for people and for teams. They can be useful for anyone who is looking for meaningful change. Our team has extensive experience planning with people receiving supports, children’s mental health, memory care, high school graduates, strategic planning, school boards, teachers, and many others.

Person Centered Thinking™ Mentor Development

Does your organization have experienced PCT trainers that would like to expand their capacities to support others in becoming PCT trainers? Nurture Care offers Mentor Training to PCT trainers who are interested in taking their expertise to the next level. Mentoring to become a PCT Mentor requires mentorship from two mentor trainers.
